The On-Site Dentist has partnered with school districts across Colorado to provide in-school dental exams, x-rays, cleanings, fluoride treatments, sealants, educational materials, and supplies at NO COST to you or the school. Having your child seen by The On-Site Dentist is easy and convenient.
The On-Site Dentist:
- Comes directly to your student’s school
- Is set up just like the hearing and vision screenings
- Coordinates all scheduling with the school
- Will help guide you to a local dentist if your student needs additional care
- Will bill Medicaid and/or your insurance for the services performed
Your child is eligible if they have not seen a dentist in over six (6) months. If you would like your child to participate in our program, please complete the permission slip and the medical history form and return it to your student’s school promptly to ensure they are seen at our next visit.
